I was bored (while waiting for my Mac to make clips of the finale...), and frustrated, and Wells ended the writing of one of the most interesting characters on TV ever by a total absence of closure, and I think it's because he maybe didn't want to disappoint anyone.
Some people like Carter with Kem, some don't... Some think Carter should run as far away from County as possible, some think he should become the ER chief there.
So... out of my boredom, I came up with a few possibilities for a Carter closure, and you can choose yours, depending on your point of view on Carter:
1) You love Carter and you are dying for a happy ending for him. You never really liked Kem though...
2) You love Carter, and find that Noah Wyle does an amazing job at portraying Carter when he is miserable and suffering. You never really liked happy Carter, because Noah is much hotter with tears in his eyes than when showing dimples.
3) You're a Carby! Carby all the way since season 1 (you knew she was gonna come one day, you just knew it the first time you saw Carter in the pilot!!!)
4) You hate Carter, never liked him. Way too much focus on him... Whiny miserable guy. One of the worst ER characters ever.
5) You love Carter and want a happy ending for him and for Kem.

1 - Carter calls Kem in the morning after working on multiple traumas in the ER. They meet for lunch, and speak for a long time, finally deciding to divorce. Carter returns as full attending at County.
A couple of weeks later, he calls Susan Lewis, asking if she'd like to meet again. She flies to Chicago right away. They get married, and Cosmo gets a half sister one year later, that they name Martini.
The Joshua Carter Center is a success. Carter is decorated with the Chicago Medal of Honor for his amazing work for the community.
Finding the name "Joshua Carter" too reminiscent of his painful past, Carter decides, along with Susan, to rename the center the "Martini and Cosmo Carter Center".

2 - After handling multiple traumas, Carter calls Kem. She refuses to meet for lunch. She later calls him from Paris, saying they should get divorced. He accepts. He returns as a full attending to County, doing many shifts to forget his pain.
He does not manage to get substantial founding for the Carter Center, and spends his own money to keep the doors open. Eighteen months after its opening, the Joshua Carter Center must close, and Carter is completely broke.
Working in the ER and highly depressed, he starts shooting up pain meds that he can find in the ER. After three months of daily high dosage of morphine, he completely screwed his donated kidney and goes back into renal failure. Given his history of drug addiction, he can't make it to the UNOS list. He asks his parents to be tested in order to donate a kidney, but they refuse.
On June 4th, 2013, Carter dies from complication of renal failure, alone at his place. His parents come back to Chicago three weeks after his funeral. They were stuck in the Bahamas, and last they heard of him on the phone, for Christmas 2011, Carter said he was fine...
His grave is visited by Peter Benton only, who named his first son with Cleo "John Truman".
John Truman Benton dies of leukemia at age 11.

He starts drinking heavily to forget his pain. But six months later, he comes back to his sense, and starts attending AA meetings.
One day, Abby Kovac shows up at one of these meetings to celebrate her two years sobriety anniversary. Carter and her share a glance while Abby is on the stage sharing her story, and butterflies start flying through the window inside the room. She leaves the stage and walks to Carter, grabs his hand and leads him out. They walk to Lake Michigan, and go skinny diving right away. The cold water makes them hungry so they go for coffee and pie.
Later that day, at Carter's place, Carter and Abby talk and Carter asks her to stay in Chicago. She tells him "I'm not going anywhere...", and he answers "Sure? Because I really need something to hang on to right now". Once again, butterflies come in, as well as pies flying through the window. Whatever...

4 - Carter gets very busy with the multiple traumas and forgets to call Kem to ask her about meeting for lunch. She leaves Chicago, thinking Carter is a real a$$hole, and she is right.
Carter leads one of the traumas, along with Gates, and keeps on yelling at Dr. Wise and other interns. Dr-know-it-all-Carter thinks they should crack open the chest of the guy on the table, while Gates and the interns disagree. Gates and Carter fight, and Gates ends up cutting Carter's carotid artery accidently with a scalpel. Carter collapses on the floor. The other doctors think "Good! we can now handle the trauma in our own way without that son of a b!tch !". They wheel the patient to the OR, leaving Carter alone bleeding to death. His body is found in the trauma room 10 hours later. Dr. Banfield calls his family to announce the news. His parents are delighted. No one wants to take care of a funeral service. They ditch his cadaver in the ambulance bay dumpster.
