Just one day!
LOVE IS BLIND—NOAH WYLE RETURNS AND COURTNEY B. VANCE GUEST STARS—It’s Valentine’s Day in the ER and Neela (Parminder Nagra) seems to be under the spell of Dr. Brenner (David Lyons). Gates (John Stamos) treats a bizarre mushroom hunter that comes in on his wedding day with breathing problems. Banfield (Angela Bassett) refuses to give up on having a baby but is having trouble convincing her husband (guest star Courtney B. Vance) of her plan. Neela gives a little girl great news about her mother’s heart transplant surgery but is disappointed when a critical mistake is made. Meanwhile, Dr. Carter (guest star Noah Wyle) makes a surprise return to the ER. Linda Cardellini and Scott Grimes also star

Done with watching now! and woow.. i'm still a bit shaky. The end was really sad. All in all, nice episode, though I'm disappointed there wasn't more of Carter, of course...
Don't read more if you don't want details yet ;-).
So we see Carter arriving in his Jeep to the ER, and he takes a glance at the ambulance bay, and he looks so deeply sad, it's heartbreaking.
Then it was mostly all the scenes already posted as preview clips. Just a tiny bit more here and there. The full time, you can see that Carter is not well. With better quality video, it really shows they made him look sick. All in all, until about the last 5 minutes, I was quite disappointed that it was 'only that'.
Then there are 2 really nice sad scenes, with nice music, of Carter checking the sign for the "Joshua Makalo Carter" center, and then standing close to Lake Michigan.
And last scene shows a row of patients on dialysis, the faces of the patients farther away hidden by the dialysis machine, and we get to see one arm, and of course, it's Carter there. He looks utterly depressed.
The dialysis is hooked up in his arm, so he has a fistula forearm access working already, meaning he is sick for a little bit, at least 6-8 weeks. That last scene with no music, just the pumping sound of the machine, is really quite shocking. I was really expecting when Noah said in an interview that by the end of the episode, we learn that there is more than 'meets the eyes', that we would see him on dialysis on the last scene, but I wasn't totally prepared for something looking so depressing...
Voilà... Let me know what you thought, when you can watch!
ETA: I liked the little touch about how we first see Carter, just his eyes in the mirror of his Jeep, and I like in the details that he still has belongings he had years ago, same bag, same Jeep, same watch. Funny to imagine WB Studio must have a box where they kept his things...

I am really not scared! I really don't imagine how they could end up with killing Carter off. It's always been said that Wells wanted to finish the show with him. That would be the most depressing ending ever! They would get big negative critics I imagine... They can't do that.
And I really imagine that they want to bring awareness about organ donation. The beginning of the season showed Pratt being an organ donor. We have the story now with the mother waiting for a heart, and I imagine she'll die, and it'll be the way for the writers to say "See... this wouldn't happen if more people had donor cards".
And I imagine they'll use Carter storyline to say: "See, you can make someone's life so much better with being informed about organ donation, with knowing where your family stands and with having letting your family know you want to be a donor". I imagine that, in the plot about organ donation, Carter's story will be the positive one.
We see now how miserable he looks with having to go through dialysis. It's more touching because it's HIM, and not any patient. And I think they can play it out really great and moving to show him hoping for a transplant and hopefully getting it. Noah is a fantastic actor, who can put so much just in his eyes. I think they can play the card of thankfulness and gratitude just through Carter's eyes.

With all those people who are saying that he's going die just because he's sick and because that's the only way to bring all the past cast to the ER.....when I read your posts I'm more optmistic And just blind people can't recognize his talent I hope they only went down to this road to inform people. Seeing how beloved is Carter's character probably it's their way to allow people to understand how crucial it is being a donator.

I must admit I'm a bit 'amused' with all those posts everywhere sounding like because he's on dialysis, he's as good as dead. Maybe I'm influenced because I had a friend in high school who had renal failure, and I never thought of her as someone dying, and she never gave that impression.
I'm happy if I can help cheer you up! Furthermore, Noah said that the ending was going to be 'very exciting'. I'm really sure he wouldn't say that if he knew his character was dying... I don't think he'd be happy with it at all and it would somehow show in interviews.

I finally saw it!!!!! And the last scene was so SAAAAAD!!! Please, save the little boy and give him a little of happynes!!! Loved the Jerry's infatuation for "John": CALL ME!!!! ;D

I'm speechless.... well, nearly. I just saw the epi., finally. I still feel tearful. He looked so, so, sad at the end. Those mournful brown eyes. Actually, I quite liked the rest and there was more of Carter than I thought there might be, but I know he'll pull through. I'm not even going to speculate, but the waiting will be Still great to see it on my birthday. Lovely present ;D ( thnaks for wishes elsewhere , guys). Off to a special birthday lunch with a friend soon, but I will keep those beautiful sad, Noah eyes with me. Thanks for the cap as a reminder
